New Patient Special
We are excited to welcome new patients to our practice! All new patients ages 18 and older will receive a complimentary take-home teeth whitening kit after completing a comprehensive exam, cleaning, and any necessary treatments. It’s our way of saying thank you for trusting us with your dental care.
Dental Insurance
At Eagle Lake Family Dentistry, we aim to make dental care as convenient as possible. We will submit all insurance forms directly to your dental benefit provider, and we are proud to be In-Network Providers for Delta Premier and Aetna. Please note that a copayment of 50% is required for crowns, and 20% for other restorative work, on the day of service. Any remaining balance that is not covered by your insurance provider will be your responsibility. For detailed information about your coverage, we recommend contacting your insurance provider directly.
Appointment Policies
If you are unable to keep your scheduled appointment, we kindly ask that you provide at least 48 hours’ notice to allow us to reschedule your time slot.
